Job summary

Olympic Medical Center is seeking a dedicated Occupational Therapist to provide home health services in Port Angeles/Sequim. The role focuses on improving quality of life, independence, and safe home environments for adults and geriatric patients.

The OT will assess needs, develop plans, and work with a multidisciplinary team to optimize patient outcomes. Requires WA OT licensure, BLS, valid driver's license, and experience in home health or geriatrics.

Qualifications

• Bachelor’s or master’s degree in occupational therapy from an accredited program.
• Licensed as an Occupational Therapist in the state of Washington.
• One year of OT experience with adults; home health or geriatric setting preferred.
• BLS certification current.
• Valid Washington driver’s license and automobile insurance.

Responsibilities

• Provide specialized OT services to adults and geriatric patients in home health settings.
• Assess patients, develop individualized treatment plans and implement interventions.
• Promote independence and home safety through therapy and education.
• Consult with the home health interdisciplinary team and other professionals.
• Document assessments, plans and progress to support the care plan.

The role involves a range of responsibilities focused on improving the patient’s overall quality of life, maximizing their functional abilities, ensuring home safety and promoting independence. This will include conducting comprehensive patient assessments, developing individualized treatment plans, implementing therapeutic interventions and providing home safety recommendations for overall improved patient quality of life and independence as much as possible.
