UnityPoint Health is seeking a Home Health Occupational Therapist to deliver skilled, evidence-based care in patients' homes. You will work with physicians and an interdisciplinary team to create and implement personalized plans, provide hands-on therapy, and educate caregivers to promote meaningful home-based outcomes.

Candidates must hold a ACOTE-accredited OT degree, have 1 year of relevant experience, and possess a valid OT license and driver's license.

Formación

• Bachelor's degree in Occupational Therapy from ACOTE-accredited program.
• 1 year of experience with a similar patient population.
• 1 year of experience with documentation in an electronic health record.

Responsabilidades

• Conduct skilled home visits to assess patient needs, functional status, and OT goals.
• Develop and update individualized OT plans of care with patients, caregivers, physicians, and the care team.
• Deliver and direct therapeutic interventions to improve independence, including adaptive equipment training.
• Monitor patient progress and adjust treatment plans, communicating changes to physicians and team.
• Educate patients and families on disease management, safety, and independence strategies at home.

Shift: Monday - Friday | 8:00 AM - 4:30 PM

As a Home Health Occupational Therapist, you deliver skilled, evidence-based care directly in patients' homes, helping individuals improve safety, independence, and daily function. Working in collaboration with physicians and an interdisciplinary care team, you develop and implement personalized treatment plans and provide hands-on therapeutic interventions grounded in best practices. You also educate and support caregivers while ensuring accurate documentation and a patient-centered approach that promotes meaningful outcomes and a high-quality care experience.

Responsibilities

• Conduct skilled home visits to assess patient needs, functional status, and occupational therapy goals using clinical evaluation and standardized tools.
• Develop, implement, and continuously update individualized occupational therapy plans of care in collaboration with patients, caregivers, physicians, and the care team.
• Deliver and direct therapeutic interventions to improve independence, including training in adaptive equipment, prosthetics/orthotics, and safe environmental modifications.
• Monitor patient progress, adjust treatment plans, and communicate changes in condition promptly to physicians and interdisciplinary team members.
• Educate patients and families on disease management, safety, medication understanding, and strategies to support independence and recovery at home.

Qualifications

Education

• Bachelor's degree in Occupational Therapy from a program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE) of AOTA, or its successor.

Experience

• One (1) year of experience caring for similar patient population as to be assigned.
• One (1) year of experience with documentation in an electronic health record.

Licensure/Certifications

• Licensed Occupational Therapist license in the state(s) of practice.
• Valid driver's license.
• Valid Auto Insurance.
• Basic Life Support (BCLS) through the American Heart Association.
