Job summary

AdventHealth in Florida seeks an Occupational Therapist for home health to coordinate and direct patient care in Pasco County. You will perform functional assessments, develop and implement care plans, educate patients and families, and ensure safe home environments.

The role requires OT credentials, ongoing documentation, and collaboration with physicians and the health team. A sign-on bonus may be available; mileage reimbursement is provided.

Qualifications

• Master's in Occupational Therapy preferred.
• 1+ years of clinical OT experience in home health ideal.
• Experience in Medicare-certified home health preferred; licensing requirements apply.

Responsibilities

• Coordinate and direct OT care for home patients with comprehensive assessment, planning and evaluation.
• Perform evaluations of patients' functional abilities and document results.
• Develop patient-specific care plans with goals in consultation with patients, families, and physicians.
• Implement care plans through direct care, coordination and supervision of the care team.
• Educate patients and caregivers on OT techniques and home programs.
• Assess home safety and infection control and coordinate resources.

Job Description:

Up to a $5,000 Sign on Bonus Available Mileage reimbursement provided: $0.76 per mile Schedule: Monday - Friday, 8am - 5pm Company provided phone and laptop/iPad Coverage Areas (one of the following): Pasco County

• Coordinates and directs the care of home patients requiring occupational therapy, providing comprehensive assessment, planning, implementation, and evaluation.
• Performs evaluations to assess patients’ functional abilities, including activities of daily living, and documents results in clinical records.
• Prioritizes and adapts home care caseloads to meet the changing needs of patients and families.
• Develops patient-specific care plans with realistic, measurable goals in consultation with patients, families, and physicians.
• Implements care plans through direct patient care, coordination, delegation, and supervision of the healthcare team.
• Selects and applies skilled interventions to achieve the best outcomes for patients, reassessing as needed.
• Educates patients and caregivers on therapy techniques, use of assistive devices, and home programs.
• Assesses the home environment for safety, infection control, and community resource needs.
• Monitors and documents patient progress, adjusting care plans as necessary to achieve goals.
• Participates in interdisciplinary care conferences and quality improvement activities.
• Communicates effectively with physicians, clinical managers, and other healthcare team members regarding patient status and care needs.
• Maintains accurate and timely documentation of all assessments, care plans, interventions, and patient progress.
• Other duties as assigned.

Knowledge, Skills, and Abilities:

• Ability to delegate tasks to appropriate personnel as indicated by skill level and professional standing
• Home Care Regulations and Third-Party Reimbursement as it impacts care delivery
• Demonstrated knowledge of occupational therapy theory, modalities, treatments, and skills
• Basic computer skills and knowledge of Microsoft Office.
• Must be able to read, write, and speak conversational English.

Education:

Master's [Preferred] Field of Study: in Occupational Therapy Work Experience: 1+ relevant clinical occupational therapy experience [Required] Recent, relevant experience in a Medicare-certified home health agency as a case-manager [Preferred] Additional Information: Education requirements for this position are determined by the licensing or certification standards set forth by the relevant certifying body. Applicants must possess the level of education mandated for eligibility to obtain and maintain the required license or certification.
