Job summary

Signature Healthcare at Home is seeking an Occupational Therapist to cover Island County and surrounding areas for our home health program. This role offers autonomy, travel to patients where they are, and a Monday–Friday day shift with self-directed scheduling.

A $10,000 sign-on bonus is available. We provide competitive pay, mileage reimbursement, and comprehensive benefits as part of our in-home care team serving Oak Harbor, Greenbank, Langley, Anacortes, and La Conner.

Qualifications

• Must graduate from ACOTE-accredited OT program.
• Must be licensed to practice in Washington state.
• Must be eligible for or have taken NBCOT exam.
• Must have access to reliable transportation and a valid driver’s license.

Responsibilities

• Conduct interdisciplinary assessment and develop a patient-centered plan of care with the patient and caregiver.
• Educate patients, families and caregivers and coordinate with other home health services.
• Evaluate functional status and home safety; identify equipment needs and support total care plan.
• Supervise OT assistants per policy and state regulations.
• Participate in discharge planning.

Job description

Signature Healthcare at Homeis seeking to add an Occupational Therapist for Island County! Now offering a $10,000 sign-on bonus!

This position would cover Island County and surrounding areas (Oak Harbor, Greenbank, Langley, Anacortes, La Conner) for home health occupational therapy services.

This position works Monday through Friday day shift, with self-directed scheduling dependent on patient needs and territory. If you enjoy a high level of autonomy, travel and meeting patients where they are, apply today!

Duties and Responsibilities

• Assumes responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative (if any), and caregiver(s).
• Provides patient, family and/or caregiver education, counseling, and refers patient to other home health services as appropriate.
• Evaluates patient’s functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.). Evaluates home environment for hazards or barriers to more independent living. Identifies equipment needs. Participates in the development of the total plan of care.
• Supervises Certified Occupational Therapy Assistants according to organization policy and state regulations.
• Participates in discharge planning for patient.

Knowledge, Skills and Experience

• Must have graduated from an OT program accredited by the Accreditation Council for Occupational Therapy Education (“ACOTE”) of the American Occupational Therapy Association (“AOTA) or any successor organization of the ACOTE.
• Must be licensed and able to practice in Washington State.
• Must be eligible for or have taken the OT certification examination administered by the National Board for Certification in Occupational Therapy (“NBCOT”).
• The ability to drive and/or have access to transportation for agency related travel (e.g. community partners, patients, etc.) is an essential function of the job. Must have and maintain a valid driver’s license, maintain automobile insurance coverage and have access to an automobile.

Preferred Qualifications:

• Clinicians with experience in Home Health or post-acute care.
• Familiarity with Homecare Homebase EMR.

Schedule, Pay and Benefits

• Full-time and Part-Time Monday through Friday available.
• Hourly $55 - 65 (Dependent on Experience) + Mileage Reimbursement.
• Medical/Dental/Vision available the 1st of the month after hire.
• Paid company holidays, 2+ weeks PTO and 401k match after 90 days.
