Job summary

La Vita Bella Home Health in Rio Rancho, NM is seeking a licensed Physical Therapist to join a compassionate, patient-centered team delivering in-home care.

You will evaluate patients, develop personalized treatment plans, teach ADLs, and collaborate with physicians and other therapists to promote independence and safety in the home setting.

Qualifications

• Proven work experience as a Physical Therapist.
• Current knowledge of treatment practices.
• Ability to manage patients with different personalities.
• Valid license to practice physical therapy.
• Graduate degree or greater in physical therapy.

Responsibilities

• Evaluate patients’ physical, cognitive, and functional abilities to identify independence needs.
• Develop individualized treatment plans based on goals and abilities.
• Provide therapeutic interventions to improve activities of daily living.
• Assist patients with bathing, dressing, grooming, meals and daily tasks.
• Teach adaptive equipment and techniques to promote independence and safety.
• Assess homes and recommend modifications to reduce fall risks.
• Provide exercises to improve strength, balance, mobility and independence.
• Educate patients and caregivers on plans, safety, and home progress.
• Monitor progress and modify plans; collaborate with physicians, nurses and therapists.
• Maintain accurate documentation of evaluations, plans, progress, and services.

Responsibilities

• Maintain accurate and timely documentation of patient evaluations, treatment plans, progress, and services provided
