Job summary

Greenhills Health and Rehabilitation Center in Hendersonville, TN is seeking a qualified Occupational Therapist to join our rehabilitation team. The OT will assess patients referred for rehabilitation, develop individualized treatment plans, and work under the direction of the physician to optimize functional outcomes.

The role requires graduation from an accredited OT program, state licensure, and experience with long-term or geriatric care.

Qualifications

• Must have graduated from an accredited Occupational Therapist program.
• Must have a valid state license and registration.
• Oversee and supervise OT Assistants.
• Experience with long-term care and/or geriatric care preferred.
• Strong documentation skills.
• Evaluations, progress notes, discharge documentation.

Responsibilities

• Assess referred patients for rehabilitation needs.
• Develop and implement treatment programs.
• Set realistic goals with patient capacity.
• Educate patients and families on therapy methods.
• Document patient condition and progress.
• Coordinate with interdisciplinary team.
