Job summary

Interim Healthcare is seeking a Home Health Occupational Therapist (OT) for full-time or PRN roles serving Charleston, Berkeley, and Dorchester Counties in South Carolina. You will help patients regain independence in their homes.

Qualifications

• Graduate of an accredited OT program and SC OT license.
• Minimum of 1 year OT experience, ideally in home healthcare.
• CPR certification.
• Valid car insurance.
• Knowledge of state and federal home health regulations.
• Maintaining accurate and up-to-date documentation.
• Good clinical judgment, strong interpersonal skills, compassionate care.
• Providing services in accordance with agency policies and regulations.

Responsibilities

• Provide occupational therapy to patients unable to perform daily tasks at home.
• Work with a home health team to follow the plan of care and personal goals.
• Assess patient, observe deficits, establish goals and document progress.
• Assist patient with exercises to improve fine motor skills and coordination.
• Suggest adaptive equipment to offer added support.
• Assess fall risks and introduce home safety strategies.
• Educate patient and family on plan of care, exercises, goals and self-care.

As a Home Health Occupational Therapist, here’s a big-picture view of what you’ll do:

• Provide occupational therapy to patients unable to perform daily tasks due to an illness or injury
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, PT and SLP, focused on the patient’s plan of care and personal goals
• Assess patient, observe deficits, establish therapy goals and document progress
• Assist patient with exercises to improve fine motor skills and coordination.
• Suggest adaptive equipment such as grab bars and shower chairs to offer added support
• Assess fall risks and introduce strategies to improve home safety
• Educate patient and family on plan of care, exercises, goals and self-care

A few must-haves for Home Health Occupational Therapists:

• Graduate of an accredited Occupational Therapy Program and active OT license in South Carolina
• Minimum of 1 years of occupational therapy experience, ideally in home healthcare
• CPR certification
• Valid car insurance
• Knowledge of state and federal home health regulations
• Maintaining accurate and up to date documentation
• Good clinical judgment, strong interpersonal skills, resourceful and compassionate
• Providing services in accordance with agency policies and regulations that define scope of practice.
