Trinity Health Chelsea Hospital in Chelsea, MI is seeking an Occupational Therapist to provide evaluation and treatment of hospital inpatients and outpatients in a range of specialty programs.

The therapist will deliver comprehensive assessments, create plans of care, educate patients and families, and document thoroughly to support safe, effective rehabilitation and outcomes. Collaboration with the care team is essential to maintain quality care across services.

Qualifications

• Education: Bachelors Degree or higher in Occupational Therapy from accredited university.
• Licensure: Licensure in the State of Michigan; NBCOT certification preferred.
• CPR Certification required.

Responsibilities

• Provides evaluation and treatment of hospital inpatients and outpatients in a range of specialty programs.
• Maintains thorough documentation of patient encounters and complies with state laws and payer requirements.
• Develops and implements individualized plans of care for patients based on deficits and goals.
• Coordinates patient care with the rehabilitation team and supervises fieldwork students as assigned by management.
• Stays current with insurance guidelines for rehabilitation services and ensures appropriate clinic workflow and productivity.

Location: Chelsea, MI

Schedule: Full-time, 64hrs bi-weekly, Day Shift POSITION PURPOSE

The Rehabilitation Services Staff Occupational Therapist provides evaluation and treatment of hospital inpatients and outpatients in a range of specialty programs.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

1. Maintains good rapport and cooperative relationships. Approaches conflict in a constructive manner. Helps to identify problems, offer solutions, and participate in their resolution.
2. Maintains the confidentiality of information acquired pertaining to patient, physicians, colleagues, and visitors to Chelsea Hospital. Discusses patient and hospital information only among appropriate personnel in appropriately private places.
3. Behaves in accordance with the Code of Conduct, Service Excellence Standards, and the Mission, Vision, Values and policies of Chelsea Hospital.
4. Assumes responsibility for performance of job duties in the safest possible manner, to assure personal safety and that of coworkers, and to report all preventable hazards and unsafe practices immediately to management.
5. Demonstrates self-directed learning.
6. Provides quality patient care by considering the age specific, development and cultural needs through competent clinical practice. Demonstrates unit/area competencies.
7. Identifies and assists in resolution of problems related to safety, sanitation and maintenance of the environment.
8. Practices effective communication in all interactions (patient, visitor, co-worker, etc.)
9. Promotes a positive work environment by demonstrating teamwork.
10. Orients patients and families to the therapeutic purposes and procedures of Occupational Therapy.
11. Provides a thorough occupational therapy evaluation and creates an appropriate plan of care based on the patient deficits.
12. Instructs patients and families in individualized home programs.
13. Participates in departmental quality assurance and improvement activities.
14. Provides for his/her own professional growth and education and contributes to staff development activities.
15. Supervises Occupational Therapy fieldwork students as assigned by the Manager or Student Program Coordinator.
16. Stays up to date on all participating insurance guidelines for rehabilitation services.
17. Provides thorough documentation on each patient interaction. Documentation must be timely and include all pertinent information required by state law, accrediting agencies and 3rd party payers.
18. Completes daily charges for each patient while adhering to ethical practices per insurance guidelines for each payer.
19. Works with Manager and secretarial staff to ensure schedules are managed well to achieve targeted productivity and clinic budget.
20. Participates in regularly scheduled staff meetings.

OTHER FUNCTIONS AND RESPONSIBILITIES

REQUIRED EDUCATION, EXPERIENCE AND CERTIFICATION/LICENSURE

• Education: Bachelors Degree or higher in Occupational Therapy from accredited university
• Experience:
• Certification/Licensure: Licensure in the State of Michigan. Certification with the NBCOT preferred.
