Job summary

Blue Stone Therapy in Sheffield, IA is seeking a licensed Occupational Therapist for a full-time permanent position. The role offers a $5,000 sign-on bonus for eligible full-time hires and a comprehensive benefits package.

You will conduct comprehensive OT evaluations, develop treatment plans, and coordinate with MD orders. The organization emphasizes mentorship, education, and career development within a supportive team.

Qualifications

• Graduate of an accredited university with a B.S., MA, or MS or Doctorate in Occupational Therapy recognized by AOTA.
• NBCOT-OTR credential (if state mandated).
• Current license/registration as an Occupational Therapist in-state as applicable.

Responsibilities

• Provide a comprehensive occupational therapy evaluation based on MD orders.
• Document findings on the standardized evaluation form timely, accurately and adhering to all guidelines.
• Provide a comprehensive treatment plan including long and short-term goals, frequency, duration, and treatment modalities.
• Ensure MD orders are obtained for evaluations, treatments, recertifications and discharges.
• Provide comprehensive treatment to patients utilizing modalities and modify patient treatment as indicated while adhering to precautions.
