Job summary

Apollo Home Healthcare is hiring a full-time or part-time Occupational Therapist to join our Home Healthcare Team and see patients in Northern California's Bay Area. You will supervise a COTA and collaborate with a team to deliver patient-centered care.

Responsibilities include evaluations, home safety assessments, and developing individualized plans while documenting progress in WellSky/Kinnser EMR. Travel to patient homes is required.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Active California Occupational Therapist license.
• Current CPR/BLS certification.

Responsibilities

• Assess patients' strength, mobility, coordination, and home safety.
• Develop and update individualized treatment plans based on goals and progress.
• Enhance independence in activities of daily living (dressing, bathing, mobility).

• Document evaluations, visits, and progress in EMR in a timely manner.
• Communicate progress and changes to the care team and physician.
• Supervise COTAs per state guidelines.
• Participate in care planning and discharge planning.

Job description

Apollo Home Healthcare is hiring a full-time or part-time/per diem Occupational Therapist to join our Home Healthcare Team and see patients in Northern California's Bay Area. In this role, you will provide high-quality occupational therapy services to patients in the comfort of their own homes, helping them regain independence and improve their daily functioning. You will supervise a Certified Occupational Therapy Assistant (COTA) and work with a team of dedicated healthcare professionals who are committed to delivering exceptional patient-centered care.

What You'll Do

• Provide in-home occupational therapy evaluations and treatments according to the physician's plan of care
• Assess patients' strength, coordination, mobility, and ability to function safely at home
• Develop and update individualized treatment plans based on patient goals and progress
• Help patients improve independence with daily activities such as dressing, bathing, mobility, and home safety
• Educate patients and caregivers on exercises, adaptive techniques, and safe movement
• Document evaluations, visits, and progress in WellSky/Kinnser in a timely manner
• Communicate patient progress, concerns, and changes in condition to the care team and physician
• Supervise COTAs in accordance with state guidelines
• Participate in care planning, case conferences, and discharge planning

Requirements

• Graduate of an accredited Occupational Therapy program (Master's or Doctorate in Occupational Therapy)
• Active California Occupational Therapist license
• Current CPR/BLS certification
• Reliable transportation and willingness to travel to patient homes
• Home Healthcare experience
• Experience documenting in the Kinnser/Wellsky EMR preferred, but not required
