• Provide comprehensive and compassionate therapy services in a sub-acute/skilled nursing facility setting.
• Collaborate with rehab therapists and nursing staff to support resident care.
• Maintain timely and accurate documentation with electronic systems.

Responsibilities

• Deliver OT services to residents in sub-acute/skilled nursing rehab.
• Work with interdisciplinary team to achieve resident goals.
• Document assessments, treatment plans and progress notes.
• Demonstrate flexibility with hours and caseload as needed.
