Renewal Rehab is seeking a licensed Occupational Therapist for a full-time role in Niles, IL. You will evaluate and treat patients with functional, cognitive, and ADL impairments, develop personalized care plans, and collaborate with a multidisciplinary team in a skilled nursing/long-term care setting.

Applicants must hold an OT degree from an accredited program, be NBCOT certified (OTR) if required by state, and hold active state licensure.

• Licensed Occupational Therapist in the state of practice.
• NBCOT certification as OTR if required by state.
• Active OT license required.

Responsibilities

• Evaluate and treat patients with functional, cognitive, and ADL impairments.
• Create and implement personalized care plans.

• Maintain documentation to meet regulatory requirements.
• Ensure physician orders are current and properly obtained.
