Job summary

The Lakes Home Care, Inc. is seeking an Occupational Therapist to join our team, delivering care in outpatient settings and patients' homes. You will diagnose initial needs and help individuals improve their daily living and working skills.

Responsibilities include assessments, personalized treatment planning, therapeutic exercises, and education for patients and families, in collaboration with the broader care team.

Qualifications

• Outpatient OT experience required.
• Solid knowledge of anatomy and physiology.
• Familiarity with medical terminology.
• Expertise in Activities of Daily Living (ADLs).
• Proficiency in patient care.
• Experience in occupational health.
• Experience in clinic settings.
• Experience in home health services.
• Experience in acute care environments.

Responsibilities

• Conduct patient assessments to determine individual needs and goals.
• Develop personalized treatment plans to improve patients' activities of daily living.
• Provide therapeutic exercises and activities to enhance motor skills and coordination.
• Educate patients and families on adaptive techniques and equipment.
• Collaborate with healthcare professionals to ensure comprehensive patient care.
