Renewal Rehab, part of Enhance Therapies, is hiring a licensed Occupational Therapist for a full-time role in a skilled nursing facility in Illinois. You will evaluate and treat patients with functional, cognitive, and ADL impairments and create personalized care plans.

Qualifications

• OT degree from an AOTA-accredited program.
• NBCOT certification as an OTR if required by the state.
• Current OT license in the state of practice.

• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements.
• Ensure all required physician orders are obtained and up to date.
