Renewal Rehab in Summer Hill, IL is hiring a full-time Occupational Therapist to join a skilled nursing facility team. IL OT license is required; NBCOT credentialing where applicable, with strong communication and a dedication to patient care.

Qualifications

• Must hold OT degree from ACOTE-accredited program.
• NBCOT-OTR certification required where state dictates.
• Current OT license in IL or eligibility to practice in IL.

Responsibilities

• Evaluate and treat patients with functional, cognitive, and ADL impairments.
• Create and implement personalized care plans.
• Collaborate with interdisciplinary team to ensure outcomes.
• Maintain accurate documentation meeting regulatory requirements.
• Ensure physician orders are obtained and up to date.
