Prava Therapy in Apex, North Carolina, is hiring a full-time Occupational Therapist to join an Independent Living community. You will provide skilled clinical care, develop personalized plans, and promote daily living independence through ADLs/IADLs and mobility training.

You'll build relationships with residents, families, and the care team, tailoring interventions to meaningful life activities and safe home navigation, while documenting outcomes and coordinating care.

Responsabilités

• Perform comprehensive evaluations and develop individualized plans of care.
• Provide skilled treatment addressing ADLs, IADLs, functional mobility, upper extremity function, adaptive strategies, cognitive-functional performance, and daily living independence.
• Connect therapy interventions to meaningful, resident-centered goals.
• Build strong relationships with residents and understand what matters most to them.
• Identify changes in function and opportunities for early intervention.
• Educate residents, families, caregivers, and community team members on home/apartment safety, adaptive equipment, energy conservation, and independence.
• Collaborate with physicians, community leadership, wellness professionals, and other members of the care team.
• Monitor progress and modify treatment plans based on clinical presentation and functional outcomes.
• Maintain timely, accurate, and compliant documentation.
• Participate in care coordination and discharge planning as appropriate.
• Contribute to a culture of clinical excellence, compassion, professionalism, and continuous learning.
