Job summary

Asian Health Services is seeking a dedicated Occupational Therapist for its PACE program to evaluate and treat seniors in the community. You will join the PACE Interdisciplinary Team (IDT) and conduct OT assessments for new and existing participants, guiding holistic care plans that address medical, physical, social, and emotional needs.

You will educate participants, caregivers, and PACE staff to maximize daily functioning, perform home visits when needed, and document progress in the PACE EHR,

Qualifications

• Master’s degree in Occupational Therapy (or Doctorate after 2000).
• Minimum 1 year work experience as an Occupational Therapist.
• Bilingual ability (Cantonese and/or Mandarin) with culturally/linguistically appropriate services.
• Experience with frail/elderly populations; home visits may be required.
• Experience with Electronic Health Records (EHRs).
• Medical clearance and up-to-date immunizations required.

Responsibilities

• Complete occupational therapy assessments for new and existing PACE participants as part of the IDT care planning.
• Develop and implement OT care plans addressing medical, physical, social, and emotional needs.
• Provide OT education to participants, caregivers, and PACE staff to maximize function.
• Document progress and notes in the PACE EHR; ensure quarterly progress reporting.

Job Summary

Essential Job Functions

• Participates as a PACE Interdisciplinary Team (IDT) member andis responsible forcompletingoccupationaltherapy assessments for new PACE participants and reassessmentsforexisting PACE participants as part of the PACE IDT care planning process.
• In collaboration with the primary care provider and IDT, develops and implements occupational therapy care plans for PACE participants,identifyingand addressing their medical, physical, social, and emotional needs.
• The occupational therapy service shall be under the direction of an occupational therapist. An occupational therapy assistant or aide shall work only under the supervision of an occupational therapist.
• The occupational therapy services staff shallworkthe hours necessary to meet the needs of each participant as specified in the individual plan of care by theinterdisciplinary team andin accordance withthe staffing requirements of the Department.
• Attends IDT and morning meetings and reports on participants'functionalstatus, including changesinindividual participants’functional status and theirabilityto perform meaningful daily activities.
• Based on clinical observation or occupational therapy reassessments,makesrecommendations to the IDT for incorporating ormodifyingoccupationaltherapy interventions to participant care plans.
• After each treatment, make notationsspecifyingthemethod of treatment and theparticipant'sreaction.Narrative notes shall be enteredinthe participant's health record whenindicatedand signed by the occupational therapist.
• Monitors PACE participants for changes in their health or psychosocial status requiring anoccupationaltherapy reassessment.Promptly communicates pertinent changes in participant status to the IDT.
• Responsible for providingoccupationaltherapy education to participants, caregivers, and PACE staff to addressfunctional issues related toperforming meaningful daily activities.Home visits may be necessary toprovide educationoroccupationaltherapy assessments/reassessmentsto participants and caregivers.
• Serves as anoccupationaltherapysubject matter expert and communicatesbest practicesfor a frail elderly populationto the IDT, participants, and caregivers.
• Completes andmaintainsdocumentation in the PACE EHRin accordance withfederal PACE regulations and organizational policy.Progress notes shall be written at least quarterly in the participant's health record and signed by the occupational therapist.
• Supports the PACE QI program and participants in PACE QI activities as assigned.

Minimum Qualifications

• Master’s degree in Occupational Therapy if obtained before 2000; Doctorate degree in Occupational Therapy if obtained after 2000.
• Minimum1-yearwork experience as anOccupationalTherapist.
• One year of experience working with a frail or elderly population, or in the absence of such knowledge, receiveappropriate trainingon working with a frail or elderly population upon hiring.
• Bilingual ability (Cantonese and/or Mandarin) and experience providing culturally and linguistically appropriate services.
• Ability to make home visit and drive to community locations as appropriate and necessary.
• Before working independently, meet a standardized set of competencies for theOccupationalTherapist positionestablishedby the PACE plan.
• Experience working in Electronic Health Records (EHRs).
• Before engaging in participant care, be medically cleared for communicable diseases and have up-to-date immunizations and vaccines.
• Have not been convicted of criminal offenses involving Medicaid, Medicare, other health insurance or health care programs, or social services programs under title XX of the Act.
• Havenot been excluded fromparticipatingin Medicare and Medicaid programs.
• Have not been convicted of criminal offensespertaining tophysical, sexual, drug, or alcohol abuse.

Certifications, Licenses, Registrations

• Doctor of Physical Therapy (DPT) degree from an accredited physical therapy program.
• Current, active California Physical Therapist (PT) license in good standing.
• Current CPR/BLS certification required.
