Job summary

Creative Health Solutions is seeking a skilled Occupational Therapy Assistant to join our team serving Skilled Nursing facilities. The OTA will work closely with therapists to develop and administer individualized therapy programs aimed at improving patients' physical and functional well-being.

You will contribute to plan-of-care implementation and collaborate with a multidisciplinary team in a supportive, patient-centered environment.

• Valid State OTA License is required.

• Collaborate with a team of healthcare professionals to create personalized treatment plans.
• Document patient progress and communicate with healthcare professionals regarding treatment plans and patient outcomes.
• Educate patients and their families on therapeutic procedures and techniques.
• Stay up to date with the latest therapy techniques and methodologies.
