FOX Rehabilitation, based in Madison, WI, is seeking licensed Occupational Therapy Assistants to provide one‑on‑one, in‑home geriatric care under supervision of an Occupational Therapist. The role centers on delivering personalized treatment and helping patients regain strength and independence in their own homes.

You’ll collaborate with an interdisciplinary team, receive mentorship from your supervising OT, and have opportunities to grow clinically and professionally in a supportive environment.

Formación

• Licensed OTAs with Wisconsin license or eligibility.
• Graduate of an accredited OTA program.
• Current CPR certification required.
• Proficient in Microsoft Office and basic computer skills.

Responsabilidades

• Document visits accurately in FOX’s EMR system.
• Coordinate with patients, POAs, and referral sources as needed.

Responsibilities

• Deliver individualized treatment plansestablishedby your supervisingOccupationalTherapist, adjusting activities based on patient response and progress
• Instruct, motivate, and safeguard patients through therapeutic exercises and functional activities,observingand tracking their progress
• Document visits accurately and on time within FOX’s EMR system, in line with FOX’s Clinical Documentation standards
• Stay connected with patients, POAs, and community referral sources, and help coordinate coverage when needed

Qualifications

• ValidOccupational Therapy Assistant license in the state(s) of practice, or eligibility to obtain
• Graduate of an accredited Occupational Therapy Assistant program
• Current CPR certification
• Basic computer skills; proficient in Microsoft Office

Physical Requirements

FOXOTAs must be able to meet the physical demands of in‑home and community‑based care, including:

• Exerting medium physical effort: lifting 25–50lbsoccasionally, 10–25lbsfrequently, and up to 10lbsconstantly
• Performing CPR
• Providing moderate to maximum assistance with patient transfers and bed mobility, including repositioning a bed‑bound patient alone
• Standing, sitting, bending, kneeling, crouching, climbing stairs, and extensive walking
• Performing repetitive movements of the upper extremities
• Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model
• Wearing PPE when required
