Job summary

Sweetwater Care is seeking an Occupational Therapy Assistant to support patient rehabilitation in a clinical setting in Willows, CA. The OTA will work under the supervision of an Occupational Therapist and participate in plan development, treatment delivery, and progress documentation.

The role requires strong communication with patients and families, accurate documentation, and adherence to regulatory requirements.

Qualifications

• Active licensure as an Occupational Therapy Assistant is required.
• Experience assisting licensed therapists in clinical settings is preferred.
• Excellent communication with patients, families and health team members is essential.
• Strong documentation skills for medical records and regulatory requirements.

Responsibilities

• Consult with patients to learn about their physical condition and symptoms and review referrals.
• Assess movement and diagnose dysfunction through observation and patient interaction.
• Develop individualized treatment plans with OT supervision.
• Coach patients and teach therapeutic exercises and massage techniques.
• Maintain accurate patient records and track goals in the clinical record.
• Utilize standardized assessments and evidence-based practice to support interventions.
• Monitor patient progress and adjust treatments as required by regulatory standards.
• Evaluate outcomes and adjust services based on patient progress and referrals.
• Provide counseling and information to patients, families, and caregivers.
• Communicate progress and plans with supervisors and health team members.
• Prepare discharge summaries in compliance with regulatory requirements.
• Document all clinical information in electronic systems and record treatment minutes.
• Participate in trainings and conferences as required.
• Supervise rehabilitation aides and students when applicable.
• Adhere to company policies, procedures, and code of conduct.
• Keep an active State Licensure.

Principal Responsibilities

The Ocupational Therapist ASSISTANT performs treatments and exercises to help patients recover from illnesses and injuries. Their main duties include using various treatment techniques to develop personalized treatment plans, To increase mobility and teaching patients exercise techniques to improve their health.

This position reports to the Occupational Therapist.

• Consult with patients to learn about their physical condition and symptoms. Review their medical history and referrals from doctors or surgeons.
• Diagnose movement dysfunction by listening to patients and observing them as they move.
• Develop individualized treatment plan
• Coach patient and teach them therapeutic massage to ease patients' pain and provide stimulation to promote healing
• Maintain patient record by keeping track of goals
• Utilize standardized assessments and evidence-based practice to support clinical interventions.
• Assesses patients' progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
• Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
• Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy. Accurately records treatment minutes for services provided per all regulatory and clinical requirements.
• Participate in facility and company required trainings, in-services and conferences.
• Able to supervise and support rehabilitation aides and/or students.
• Adherence to Company required policy and procedures and code of conduct.
• Active State Licensure
