Creative Health Solutions, LLC is seeking a passionate PRN Occupational Therapy Assistant for our Skilled Nursing facility with Assisted Living in Winter Haven, FL. The OTA will implement treatment plans, collaborate with a professional team, and document patient progress to support optimal outcomes.

The ideal candidate will educate patients and families on therapy procedures, stay updated with evolving techniques, and contribute to a caring, patient-centered rehab environment.

Qualifications

• Valid State OTA License is required.
• Experience in Skilled Nursing/rehab settings preferred.
• Strong communication and documentation skills.
• Ability to educate patients and families about therapy.

Responsabilités

• Implement treatment intervention plans addressing goals on the care plan.
• Collaborate with healthcare professionals to create individualized therapy plans.
• Document patient progress and communicate outcomes to the team.
• Educate patients and families on procedures and techniques.
