Job summary

FOX Rehabilitation in Ann Arbor, MI is seeking licensed Occupational Therapists to provide one-on-one OT care in patients’ homes or senior living settings. You will evaluate, plan, and deliver evidence-based therapy while collaborating with an interdisciplinary team.

Telehealth may be incorporated to extend access and flexibility, enabling meaningful patient outcomes. This role emphasizes autonomy, flexible scheduling, and building lasting patient relationships within a clinician-operated

Qualifications

• Valid Occupational Therapy license in the state(s) of practice.
• Degree from an accredited Occupational Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one Occupational therapy to older adults in homes or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, mobility, and independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.

Physical Requirements

FOX clinicians must be able to meet the physical demands of in-home and community-based care, including:

• Exerting medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
• Performing CPR
• Providing moderate to maximum assistance with patient transfers and bed mobility
• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
• Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model
• Wearing PPE when required
