Job summary

Assured Home Health, part of the LHC Group within the Optum family, seeks a Per Diem Occupational Therapist in Oregon. You will assess functional status, ADLs, and home safety, and collaborate with the care team to implement therapy plans in patients’ homes.

You will coordinate with physicians, provide instruction to patients and families, and adapt therapy approaches to diverse home environments, while maintaining quality patient care and documentation.

• Current Occupational Therapy licensure in Oregon.
• Current driver's license and access to a vehicle or reliable public transport.

As the Per Diem Occupational Therapist, you will be responsible for assessment and evaluation of patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments and adaptive equipment, and other occupational therapy needs as defined by medical conditions.

Primary Responsibilities:

• Evaluates the patient's functional status, status of all body systems as required for CMS documentation, and occupational therapy needs. Consults with the physician in the development of the therapy plan of care
• Observes, records, and reports to the nurse supervisor and the physician the patient's response to treatment and changes to the patient's condition
• Conducts patient assessments evaluating the level of function by applying diagnostic and prognostic functional ability tests
• Instructs patient, family, caregiver, and participating members of the health care team in the areas of therapy in which they can participate to assist the patient
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

Required Qualifications:

• Current Occupational Therapy licensure in state of Oregon
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation

Preferred Qualifications:

• Current CPR certification or ability to obtain with 90 days of employment
• Demonstrate ability to manage multiple tasks simultaneously
• Demonstrate ability to work independently
• Good communication, writing, and organizational skills

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $90,904 to $136,356 annually based on full-time employment. We comply with all minimum wage laws as applicable.
