Job summary

Silas Creek Rehabilitation Center in Winston-Salem, NC seeks a dedicated Occupational Therapist to join our rehabilitation team on a full-time basis. You will work with an interdisciplinary team in a skilled nursing and long‑term care environment, evaluating Residents and creating individualized therapy plans.

Responsibilities include implementing OT services per physician orders, monitoring progress, documenting accurately, and educating Residents and families about goals and progress.

Qualifications

• Qualified Occupational Therapist licensed to practice in North Carolina
• Experience working in skilled nursing or long‑term care settings preferred
• Strong communication, organization and teamwork skills required

Responsibilities

• Evaluate Residents and develop individualized treatment plans
• Implement OT services per physician orders and care plans
• Monitor progress and adjust treatment plans as needed
• Maintain accurate and timely documentation
• Collaborate with nursing, therapy and interdisciplinary teams

Silas Creek Rehabilitation Center is seeking a dedicated and compassionate Occupational Therapist (OT) to join our growing rehabilitation team. This full‑time opportunity is ideal for a motivated therapist who is passionate about improving the quality of life and independence of our Residents through individualized therapy programs.

As an Occupational Therapist, you will work collaboratively with an interdisciplinary team to provide exceptional care in a skilled nursing and long‑term care environment.

Schedule

• Weekend rotation as needed

Occupational Therapist Responsibilities

• Evaluate Residents and develop individualized treatment plans
• Implement occupational therapy services in accordance with physician orders and care plans
• Monitor Resident progress and adjust treatment plans as needed
• Maintain accurate and timely documentation
• Collaborate with nursing, therapy, and interdisciplinary team members
• Educate Residents and families regarding therapy goals and progress
• Ensure compliance with federal, state, and facility guidelines

Qualifications

• Bachelor’s or Master’s Degree in Occupational Therapy
• Current North Carolina Occupational Therapy license required
• NCBOT certification/registration required
