Job summary

STAT Home Health is seeking a PRN Occupational Therapist to provide home-based OT services in Sibley, LA. You will assist physicians with functional assessments, contribute to the care plan, and perform skilled OT interventions at patients’ homes, with regular documentation to the physician every two months.

The role requires an active Louisiana OT license and AOTA registration, CPR certification, and at least one year of OT experience.

Qualifications

• Current Louisiana OT license and AOTA registration.
• Minimum 1 year of Occupational Therapist experience.
• CPR certification required.
• Excellent patient service orientation and strong time management skills.

Responsibilities

• Assist the physician in evaluating the patient’s functional status and OT needs; help develop the plan of care.
• Provide OT services within scope of practice as defined by Louisiana laws, per plan of care and in coordination with the health care team.
• Deliver skilled OT services at patients’ homes.
• Prepare written summaries to the physician every 60 days.
• Set rehab goals for patients.
• Guide patients in therapeutic, creative and self-care activities to improve function.

Job Duties:

• Assist the physician in evaluating the patient’s functional status and occupational therapy needs, and assist in the development of the plan of care
• Provide services within the scope of practice as defined by the state laws governing the practice of occupational therapy, in the accordance with the plan of care, and in coordination with other members of the health care team
• Provide skilled Occupational Therapy at patients homes
• Prepare written summary to the physician at least every two months/sixty days
• Set rehab goals for patients
• Guide the patient in the use of therapeutic, creative and self-care activities for the purpose of improving function.

Qualifications:

• Must be currently licensed by the state of Louisiana and registered by the American Occupational Therapy Association.
• Have at least one year of experience as an Occupational Therapist (OT)
• CPR certification
• Patient service-oriented and time management skills.
