River Bend Healthcare in Seguin, TX is seeking an Occupational Therapist for a full-time role to evaluate and treat patients with cognitive and ADL needs, and to develop and implement comprehensive care plans in collaboration with the health team.

You will screen and document patient progress, discharge planning, and participate in program expansion such as Abilities Care, ACP programs, wound care, and related therapies, while maintaining regulatory and clinical standards.

Responsibilities

• Screen/evaluate patients with cognitive and/or ADL disorders and develop care plans.
• Use standardized assessments and evidence-based practice.
• Provide rehabilitative, skilled treatment per clinical standards.
• Administer ADL retraining, therapeutic activities, exercises, modalities, and assistive devices.
• Assess progress and adjust treatments in the clinical record.
• Evaluate outcomes and modify services according to referrals.
• Provide counseling to patients, families, and caregivers.
• Communicate progress with supervisor and health team.
• Prepare comprehensive discharge summaries.

OCCUPATIONAL THERAPIST – FULL TIME

River Bend Healthcare – Seguin, TX

We are currently seeking a Full Time Occupational Therapist to join our crew of in-house therapists at River Bend Healthcare and help us expand therapy programming to include Abilities Care (cognition), ACP programs, Big and Loud and wound care. As a therapy professional, you will have the freedom to create and implement additional programs to best meet the needs of the patients in this facility.

Key Responsibilities

• Effectively screens/evaluates patients with cognitive and/or ADL physical functioning disorders and develops appropriate plan of care following all regulatory and clinical practice standards.
• Utilizes standardized assessments and evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
• Administers various procedures as part of the rehabilitation plan including but not limited to: ADL retraining; therapeutic activities; therapeutic exercises; modalities; use of supportive and assistive devices; etc.
• Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
• Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
