Key Responsibilities

• Effectively screen and evaluate patients with cognitive and/or ADL functioning disorders and develop an appropriate plan of care following all regulatory and clinical practice standards.
• Utilize standardized assessments and evidence‑based practice to support clinical interventions.
• Provide rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL functioning disorders per regulatory and clinical practice requirements.
• Administer various procedures as part of the rehabilitation plan including, but not limited to: ADL retraining; therapeutic activities; therapeutic exercises; modalities; use of supportive and assistive devices.
• Assess patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluate treatment outcomes for effectiveness; modify services based on evaluation and refer as needed.
• Provide consultation and counseling to patients, families, caregivers and other service providers related to the physical disorders.
• Effectively communicate with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• Provide comprehensive discharge summary of services provided per regulatory and clinical requirements.
