Job summary

BrightSpring Health Services is seeking an experienced Occupational Therapist for our Home and Community Rehabilitation Program in Sacramento, CA. You will deliver functional rehab in real-life settings—home, school, work, and community—helping patients regain independence.

You will perform evaluations, develop and adjust treatment plans, collaborate with the care team, and support families. A CA OT license, a bachelor’s degree, CPR, and at least one year of OT experience are required; neuro

Qualifications

• Bachelor’s degree in Occupational Therapy from an accredited program.
• Current, unrestricted license as an Occupational Therapist in the state of practice (CA).
• Current CPR Certification.
• Minimum 1 year OT experience.
• Knowledge of rehab techniques for neurological diagnoses preferred.
• Excellent verbal and written communication.
• Ability to lift 50 pounds; up to 100 pounds with safety procedures.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly reassess treatment efficacy; attend staff meetings and family conferences.
• Perform discharge evaluations and provide home/community safety recommendations and resources.
• Communicate patient needs and progress to the treatment team, physician, and family.
• Coordinate with other disciplines to ensure collaboration and optimal patient outcomes.

Responsibilities

What you will do: Responsibilities listed include but not limited to:

• Perform evaluations and develop treatment plans
• Regularly reassess effectiveness of treatment plans, attend staffing meetings and family conferences
• Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources
• Communicate patient’s needs and progress to the treatment team, physician, person receiving services and family members
• Communicate with other disciplines to ensure collaboration, coordination of care and enhance patient outcomes

Qualifications

What you will need:

• Minimum of a bachelor’s degree in Occupational Therapy from an accredited program
• Current, unrestricted license as an Occupational Therapist in the state of practice
• Current CPR Certification
• A minimum of one year’s work experience as an Occupational Therapist
• Knowledge of rehabilitation techniques related to complex neurological diagnoses preferred
• Effective verbal and written communication
• Ability to lift 50 pounds
• Moving, lifting, or transferring of patients may involve lifting up to 100 pounds following safety procedures
• Duties require fine motor skills, visual acuity, and walking/standing for extended periods
• Additional physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and frequent positioning
• A health screen or examination may be required prior to assignment and periodically thereafter, per location policy and regulations
