Job summary

The Creekside Center for Rehabilitation and Healing is seeking an Occupational Therapist to join our team on a part-time basis. You will assess referred patients, design personalized therapy plans, and supervise OT assistants to deliver quality geriatric rehabilitation.

Applicants should have graduated from an accredited OT program, hold an active license, and demonstrate strong documentation. The role emphasizes collaboration with multi-disciplinary teams and commitment to patient-centered care.

Qualifications

• Must have graduated from an accredited OT program.
• Must hold a valid OT license and registration for the state of practice.
• Experience in long-term care or geriatric care preferred.
• Strong documentation/writing skills.
• Ability to evaluate and document progress, discharge summaries.
• Professionalism and timely adherence to schedules.

Responsibilities

• Assess referred patients' medical condition, functional capabilities, limitations and rehabilitation potential.
• Establish and administer a treatment program with goals based on patient capacity and physician direction.
• Set realistic and achievable goals, document and record patient condition, educate patients and families about therapy methods.
• Oversee and supervise Occupational Therapy Assistants.
• Collaborate with all disciplines to provide patient-centered care.
• Ensure confidentiality and HIPAA compliance.
• Maintain timely scheduling and professional conduct.

Occupational Therapist Part Time

The Creekside Center for Rehabilitation and Healingislooking for a talented and hard-working Occupational Therapist to join our ever-growingteam.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patients condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.

• Must have a valid professional license and registration for state of practice.

• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.

• Experience with long-term care and/or geriatric care is preferred but not limited to.

• Well versed with documentation writing

• Evaluations, weekly progress notes, re-certification and discharge documentations.

• Collaborates with all disciplines to provide best patient centered treatment and care.

• Exceptional patient service skills and ability to demonstrate clinical excellence

• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.

• Perform all other duties assigned by Director of Rehabilitation

• Compliance with confidentiality, HIPPA and Federal Residents Rights.
