Job summary

Renewal Rehab is seeking an Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute care settings. You will assess functional abilities, create evidence-based treatment plans, and deliver interventions that promote independence, safety, and quality of life.

You’ll collaborate with an interdisciplinary team, document accurately, and educate patients, families, and staff. Mentorship and growth opportunities are available.

Qualifications

• Active OT license in Ohio or eligible
• Experience in SNF/post-acute care is preferred
• Strong documentation and communication skills
