Job summary

Angels Care Home Health in Oklahoma is seeking an Occupational Therapist to perform clinical assessments, develop and revise Plans of Care, and coordinate patient care in home health settings.

You will work with a part-time schedule and enjoy benefits including health, dental, and vision insurance, generous PTO, vehicle allowance, on-call pay, CEU access, and 401(k) retirement options. Two years of OT experience and an active license are required.

Qualifications

• Current state license as an Occupational Therapist.
• Minimum two years of OT experience.
• CPR and Hepatitis profile required.
• Knowledge of durable medical equipment (DME) helpful.

Responsibilities

• Perform clinical assessment; Identify patient needs and appropriateness of patient.
• Document, prepare, and complete a Plan of Care, progress notes, and other clinical record documentation on an ongoing basis.
• Revise the POC based on evaluation and ongoing assessment.
• Apply infection control and universal precautions in patient care.
• Accept assignments consistent with education and competence.
• Use effective interpersonal relations and communication skills with the team.
• Assess physical, social, and cultural components of the patient’s occupational environment.
