Job summary

Elara Caring is seeking an Occupational Therapist to join our home-based care team. You will evaluate patients, develop personalized plans, deliver therapy to improve daily living and fine motor skills, and educate patients and caregivers on home programs and adaptive techniques.

You will monitor progress, adjust plans, and communicate changes to physicians. Supervisory duties include guiding COTAs and other staff, with travel between patient homes as needed.

Qualifications

• Current, unrestricted OT license valid for the state of work.
• Graduate of an accredited Occupational Therapy program.
• Minimum one year of experience in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
