Job summary

Quality Home Health Care SVS of MI is seeking an Occupational Therapist to provide in-home occupational therapy services. Therapists administer therapy to patients at their residence under physician orders and care plans, following supervision by the Director of Clinical Services/Clinical Manager.

The role focuses on evaluating patient needs, developing tailored therapy plans to achieve mobility goals, and collaborating with the patient’s care team while maintaining accurate documentation.

Qualifications

• Must hold the state license for occupational therapy.

Job Summary

We are seeking an Occupational Therapist to join our team! An Occupational Therapist (OT) administers occupational therapy to patients on an intermittent basis in their place of residence, This is preformed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Sercices/Clinical Manager.

Responsibilities

• Meet with patients from many different backgrounds and compassionately discuss their health concerns and therapy needs
• Conduct evaluations to determine a starting point for each patient
• Create a therapy plan specific to each patient that will help them meet their mobility goals
• Work closely with other members of each patient’s care team to provide adequate follow-up care
• Maintain excellent records of patient therapy and progress
