Job Summary

The Occupational Therapist is responsible for age‑appropriate interventions for the client that meet physical, emotional, developmental, psychosocial and educational needs for patients who require rehabilitative care. The Occupational Therapist collaborates with all disciplines throughout the episode of care to ensure all goals are met. The Occupational Therapist delivers high‑quality continuous care, providing assessment, therapeutic exercises, education, and treatments in the patient’s home. The therapist participates in a preceptor role for new employees and works in cooperation with other disciplines. The therapist is responsible for attending in‑services, staff meetings, special projects and continuing education.

Core Responsibilities And Essential Functions
  • Direct Patient Care: manage patient’s rehabilitative needs, schedule visits per MDs order, perform routine occupational therapy procedures, meet productivity standards, and seek leadership guidance as needed.
  • Coordination of Services: develop and revise the patient’s plan of care within MD orders, coordinate with the Clinical Supervisor and other team members, perform duties assigned by Home Health Leadership, and maintain communication with MDs.
  • Patient/Caregiver Education: foster caregiver support, report concerns affecting safety, utilize verbal and written tools to promote independence during recovery.
  • Staff Education and Professional Development: participate in orientation and education of new employees, attend continuing education and in‑services, and remain current in clinical knowledge.
  • Communication and Documentation: complete skilled intervention documentation within established timeframes, communicate patient status changes, apply regulatory guidelines, participate in care team conferences, and check voice mail and secure messaging daily.
  • Regulatory Guidelines: demonstrate knowledge of CMS/State and Joint Commission guidelines, accurately complete visit documentation in the EMR, and meet all documentation timeframes.
Required Minimum Education
  • Graduate of an accredited school of Occupational Therapy.

  • Basic Life Support 1.00 Required
  • BLS – Instructor 1.00 Required
  • BLS – Provisional 1.00 Required
  • Occupational Therapist 2.00 Required
  • Georgia Driver’s License 3.00 Required
  • Private Vehicle Insurance 4.00 Required

Required Minimum Experience

Home Health Experience Preferred. Minimum 1 year of full‑time Occupational Therapy experience required.
