Job summary

Paradigm Healthcare in Houston, TX is seeking a Licensed Occupational Therapist (OT) for per diem positions. You’ll work in a patient-centered SNF/post-acute setting, focusing on independence and quality outcomes.

Responsibilities include designing therapy plans, collaborating with nurses and other therapists, and educating residents and families. Flexible schedules (full-time, part-time, PRN) are available, with opportunities for mentorship and professional growth within a supportive leadership

Qualifications

• Licensed Occupational Therapist (OT) in good standing.
• Passion for helping others regain independence and confidence.
• Strong communication and documentation skills.
• SNF or post-acute experience preferred, but new grads welcome with mentorship support.

• Help residents regain independence in daily activities like dressing, bathing, mobility, and cognition
• Design personalized therapy plans that reflect each resident’s goals, not just diagnoses
• Use evidence-based interventions to drive meaningful functional outcomes
• Partner with nurses, PTs, SLPs, and physicians as part of a strong interdisciplinary team
• Educate residents and families so progress continues beyond therapy sessions
