Job summary

Hertford Rehabilitation and Healthcare Center in Hertford, NC is seeking a dedicated Occupational Therapist to join our team. You will perform comprehensive OT evaluations, create individualized treatment plans, and guide patients toward independence and improved function.

You will work with an interdisciplinary team in a supportive environment, with opportunities for professional growth, flexible scheduling, and a focus on quality patient care and family-centered outcomes.

Qualifications

• OT degree from an accredited program recognized by AOTA.
• Current OT license/registration in North Carolina or applicable state.
• SNF experience preferred.

Responsibilities

• Provide comprehensive occupational therapy evaluations and develop treatment plans with goals, frequency, duration, and modalities.
• Document findings and progress in standard forms accurately and timely.
• Collaborate with the interdisciplinary team to implement individualized care plans.
• Provide OT interventions and modify treatment as indicated.
• Maintain accurate records and participate in facility and rehab meetings.

Education

Job Summary:

Hertford Rehabilitation and Healthcare Center, a Long Term Care facility in Hertford, NC, is seeking a dedicated and compassionate Occupational Therapist to join our growing team. As an Occupational Therapist at our facility, you will have the opportunity to work with a diverse range of patients, providing comprehensive occupational therapy evaluations, treatment plans, and interventions to help individuals regain their independence and reach their full potential.

Responsibilities:

• Provide comprehensive occupational therapy evaluations based on MD orders
• Document findings on standardized evaluation forms timely, accurately, and adhering to all guidelines
• Develop and implement individualized treatment plans, including long and short-term goals, frequency, duration, and treatment modalities
• Provide occupational therapy interventions to patients, utilizing various modalities and modifying treatment as indicated
• Document patient participation and progress on progress notes accurately and timely
• Collaborate with the interdisciplinary team to develop and implement individualized care plans
• Participate in facility and rehab meetings as indicated
• Maintain accurate and complete records of patient care and services rendered

Requirements:

• Graduate of an accredited university with a B.S., MA, or M.S. or Doctorate in Occupational Therapy recognized by the AOTA
• SNF experience preferred
• Current license and/or registration in the state (as applicable)
