Job summary

Amedisys is seeking an Occupational Therapist to deliver home-based OT services under physician-approved plans of care in Hayward, CA. The role emphasizes safe, effective care and thorough documentation while supporting patients at home.

Qualifications

• Current license to practice occupational therapy in the state you are assigned to work.
• ACOTE-accredited OT program (degree from program recognized by ACOTE).
• Eligible to take or completed NBCOT entry-level certification exam.
• One year related OT experience, or approval by regional clinical leaders for mentoring/residency if less than 1 year.
• Current CPR certification.

Responsibilities

• Deliver all aspects of OT service delivery per physician-approved plan of care.
• Ensure plan of care addresses all problems identified in the assessment and documents rationale if not addressed.
• Accountable for safety and effectiveness of OT service delivery process.
• Complete documentation timely and accurately.
• Supervise home health aide when RN is not active in the case.
• Other duties as assigned.

Qualifications

• Current license to practice occupational therapy specific to the state you are assigned to work.
• Graduate of an occupational therapy program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE) of the American Occupational Therapy Association, Inc. (AOTA), or successor organizations of ACOTE.
• Eligible to take, or has successfully completed, the entry-level certification examination for occupational therapists developed and administered by the National Board for Certification in Occupational Therapy, Inc. (NBCOT).
• One year related occupational therapy experience to ensure knowledge and skills are sufficient to safely provide occupational therapy services to patients.
  • If less than one (1) year experience, then approval from regional clinical leaders required as well as appropriate mentoring and/or residency program established.
• Current CPR certification.
