Job summary

Angels Care Home Health in Emporia, KS is seeking an Occupational Therapist to provide compassionate home health care to patients. You will perform clinical assessments, identify patient needs, and develop a Plan of Care tailored to each patient.

Responsibilities include documenting progress notes, revising the POC as needed, coordinating care with the physician and team, and applying infection control practices in home settings. Licensure, CPR, and hepatitis profile are required.

Qualifications

• Current state license as an Occupational Therapist.
• Two years of experience as an Occupational Therapist.
• Current CPR and Hepatitis profile.

Responsibilities

• Perform clinical assessment and identify patient needs.
• Prepare and revise Plan of Care and progress notes.
• Document clinical records and updates to the POC.
• Coordinate infection control and universal precautions in patient care.
• Assess patient’s physical, social, and cultural environment.
