Job summary

Hertford Rehabilitation and Healthcare Center is seeking an Occupational Therapist to join our dedicated team and provide high-quality, patient-centered care to residents.

You will conduct OT evaluations, develop plans to promote independence, document progress, and collaborate with an interdisciplinary team. SNF experience and a state license are preferred to ensure compliant, effective care.

Qualifications

• Graduate of an accredited university with a B.S., M.A., or M.S. or Doctorate in Occupational Therapy.
• Current license and/or registration in the state (as applicable).
• SNF experience preferred.

Responsibilities

• Provide comprehensive occupational therapy evaluations and treatments to residents, as ordered by the physician.
• Develop and implement individualized treatment plans to promote independence and well-being.
• Document patient progress and outcomes in a timely and accurate manner.
• Collaborate with the interdisciplinary team to ensure optimal care and treatment.
• Participate in facility and rehab meetings, as indicated.
• Supervise junior staff members and students, as needed.
