Responsibilities

• Perform initial occupational therapy evaluations and ongoing reassessments.
• Develop and implement individualized treatment plans based on physician orders and patient needs.
• Provide skilled occupational therapy services in the patient's home.
• Educate patients, families, and caregivers on home exercises, safety, and ADLs.
• Coordinate with physicians, nurses, and case managers to ensure continuity of care.
• Prepare and submit clinical documentation and progress notes in a timely manner.

Position Summary

The Occupational Therapist (OT) provides skilled occupational therapy services to patients in their place of residence in accordance with physician orders and the established plan of care. The OT collaborates with physicians, caregivers, and interdisciplinary team members to improve patient safety, independence, and overall functional outcomes.

Qualifications

Required:

• Graduate of an accredited Occupational Therapy program
• Current Occupational Therapist license in the State of Colorado
• Current CPR certification or ability to obtain upon hire
• Valid driver's license, reliable transportation, and automobile insurance

Preferred:

• Home Health experience
• OASIS experience
• Two (2) or more years of Occupational Therapy experience

Essential Job Duties

• Perform initial occupational therapy evaluations and ongoing reassessments.
• Develop and implement individualized treatment plans based on physician orders and patient needs.
• Provide skilled occupational therapy services in the patient's home.
• Evaluate patient functional abilities and recommend appropriate interventions.
• Educate patients, families, and caregivers on home exercise programs, safety techniques, adaptive equipment, and activities of daily living (ADLs).
• Collaborate with physicians, nurses, case managers, and other healthcare professionals to coordinate patient care.
• Prepare and submit clinical documentation, evaluations, progress notes, reassessments, recertifications, and discharge summaries in a timely manner.
• Complete OASIS assessments, transfers, resumptions of care, recertifications, discharges, and significant change assessments according to agency requirements.
• Utilize ICD-10 diagnoses and coding appropriately.
• Monitor patient progress and communicate changes in condition to the physician and clinical leadership.
• Supervise Certified Occupational Therapy Assistants (COTAs) and Home Health Aides (HHAs) as assigned and in accordance with regulatory requirements.
• Participate in interdisciplinary care conferences, peer review activities, quality assurance, performance improvement initiatives, and staff meetings.
• Maintain compliance with all federal, state, agency, Medicare, and accreditation standards.
• Observe patient confidentiality and safeguard all protected health information.
• Maintain current CPR certification and professional licensure.
• Perform other duties as assigned.
