Job summary

Discovery at Home LLC is seeking a licensed Occupational Therapist to conduct initial evaluations, develop and implement treatment plans, and monitor patient progress in home health and outpatient settings in Florida. The role requires Medicare/Medicaid knowledge, CPR certification, and a valid driver’s license.

One year of home health experience and strong communication skills are essential. PRN opportunities available; comprehensive benefits for full-time staff and flexible scheduling.

Qualifications

• OT degree from an accredited program.
• Must be licensed or registered with the applicable State without restrictions.
• One year of home health experience required.
• Knowledge of Medicare and Medicaid guidelines.
• Must have a valid driver’s license and reliable transportation.
• Current CPR certification.
• Excellent verbal and written communication skills.
• Basic computer skills; ability to use Microsoft Windows.

Responsibilities

• Conduct initial evaluations to assess patients' needs and develop individualized treatment plans
• Develop and implement appropriate treatment plans based on patient needs and goals
• Monitor patient progress and adjust treatment plans as necessary
• Educate patients and caregivers on proper techniques to improve mobility, reduce pain, and prevent injuries
• Maintain accurate and timely documentation of patient progress
• Collaborate with other healthcare professionals to ensure patients receive comprehensive care
• Provide home health and/or outpatient services as needed, including supervisory visits, to ensure optimal outcomes
