Job summary

Fusion Physical Therapy Partners’ Engage Senior Therapy is hiring a compassionate Occupational Therapist to serve geriatric patients across assisted living facilities in Bedford, NH. The OT will assess needs, create individualized care plans, and collaborate with a diverse care team to improve daily living activities and quality of life.

You’ll lead wellness activities, educate families, and ensure compliant documentation using EMR.

Qualifications

• Graduate of an accredited occupational therapy program.
• Active or pending state license to practice occupational therapy.
• Passion for delivering quality, evidence based patient care.
• Superior customer service and interpersonal communication skills.
• Experience or interest in geriatrics, with skills in adaptive equipment and ADL training.
• Strong communication and interpersonal skills to work effectively with patients, families, and colleagues.

Responsibilities

• Conduct patient evaluations to assess functional abilities, daily living skills, and cognitive performance.
• Develop and implement individualized care plans focusing on ADLs and functional mobility.
• Lead wellness initiatives and group fitness classes.
• Collaborate with an interdisciplinary team including PTs, SLPs, and nursing staff.
• Educate patients and families about strategies to improve independence and quality of life.
• Document patient care services in EMR in compliance with state and facility standards.

Engage Senior Therapy , a proud member of Fusion Physical Therapy Partners, is now hiring a passionate and patient-focused Occupational Therapist at our partner Assisted Living Facilities in Bedford, NH. In this role, you will determine appropriate occupational therapy care and treatment for a geriatric population at a wide range of functional levels.

As an Occupational Therapist, you will:

• Conduct patient evaluations to assess functional abilities, daily living skills, and cognitive performance
• Develop and implement individualized care plans focusing on activities of daily living (ADLs), functional mobility, and other goals that matter to your patients
• Lead wellness initiatives and group fitness classes to encourage healthy habits and social engagement among patients
• Collaborate with an interdisciplinary team including physical therapists, speech therapists, and nursing staffto support optimal patient outcomes and quality care
• Educate patients and families about strategies to improve independence and better quality of life
• Document patient care services in compliance with state and facility standards using EMR
• Deliver care at one or more assisted living locations, providing meaningful services to a diverse group of seniors
