Qualifications

• Minimum two years of experience in primary care medicine in an outpatient setting preferred.
• Current licensure in the State of Illinois, DEA, Controlled Substance and CME requirements.
• Knowledge of applicable rules, regulations and standards related to quality assurance, safety and infection control required.

Responsibilities

• Provide full scope of services in line with provider's training.
• Diagnose, treat, counsel, and coordinate care for each patient.
• Fulfills on-call responsibilities and prescribes medication according to guidelines.
