Job summary

Charles B. Wang Community Health Center seeks a Registered Dietitian to partner with primary care and weight management teams to improve nutrition-related outcomes. You will assess dietary needs and design nutrition education programs for individuals and groups within the community.

The role involves developing educational materials, coordinating with the Health Education department, and supporting care teams with diabetes management and chronic disease care in a community health setting.

Qualifications

• NY State Certified Dietitian license and registration are required.
• Registration by the Commission on Dietetic Registration is required.
• Diabetes Educator Certification is preferred.
• Experience in clinical diabetes management – ideally in a community setting.
• Bilingual in English/Chinese is required.

Responsibilities

• Collaborate with primary care and weight management providers to manage patients with nutrition-related conditions.
• Assess nutritional needs and food patterns of patients, families, and the community.
• Educate and counsel children and adults on nutrition to improve and maintain health.
• Plan and provide individual and group instruction based on patient needs assessments.
• Develop nutrition educational materials and educate the community in collaboration with the Health Education department.
• Lead with nutritionists to develop a Quality Assurance program and peer review.

Job description

• Assess nutritional needs and food patterns of patients, families, and the community
• Educate and counsel children and adults on nutrition to improve and maintain health
• Plan and provide individual and group instruction based on patient needs assessments
• Develop nutrition educational materials and educate the community on nutrition topics in various formats in conjunction with the Health Education department
• Lead and coordinate with nutritionists to develop nutrition Quality Assurance program, Peer
• Review, and other quality improvement activities, update and review nutrition documentation template in EMR periodically
• Assist in supervisory and administrative duties of other nutritionists
• Assist other department heads in supervision and teaching of staff in delivering nutrition education and counseling
• Collaborate with care team to provide care management to patients with chronic diseases and high-risk patients.
• Organize nutrition events with nutritionists and other staff
• Assist with collection and analysis of obesity and diabetes data and reports
• Assist research team on grant applications, data collection and analysis and clinical publications
• Provide training for other health center nutritionists, healthcare interns and Project Ahead students
• Assist with validation of UDS report and other relevant reports
• Other duties as assigned

Qualifications

• Current New York State Certified Dietitian license and registration
• Certification by the Commission on Dietetic Registration
• Diabetes Educator Certification preferred
• Previous clinical diabetes management experience – preferably in community setting
• Bilingual in English/Chinese required
