Job summary

Charles B. Wang Community Health Center in Flushing, NY is seeking a full-time Registered Dietitian to support nutrition care across the patient population.

This role involves collaborating with primary care and weight-management providers, assessing patient needs, and delivering nutrition education to individuals and groups. The successful candidate will develop and share nutrition materials, participate in quality assurance activities, and help update EMR documentation.

Qualifications

• Current New York State Certified Dietitian license and registration.
• Certification by the Commission on Dietetic Registration.
• Diabetes Educator Certification preferred.

Responsibilities

• Work with care teams to manage nutrition-related conditions.
• Assess nutritional needs of patients, families, and the community.
• Educate and counsel patients on nutrition to improve and maintain health.
• Plan and provide individual and group nutrition instruction based on needs assessments.
• Develop nutrition education materials and educate the community in conjunction with Health Education.
• Lead and coordinate with nutritionists to develop nutrition QA programs and other quality improvement activities.
• Review and update nutrition documentation templates in EMR periodically.
• Assist in supervisory and administrative duties of other nutritionists.
• Collaborate with care team to provide care management for chronic diseases and high-risk patients.
• Organize nutrition events with staff
• Assist with data collection and analysis for obesity and diabetes reports.
• Assist research team on grant applications, data collection and publications.
• Provide training for other nutritionists and healthcare interns.
• Assist with validation of UDS reports and other relevant reports.
• Other duties as assigned.

Education

New York StateCertified Dietitian license and registration

Certification by the Commission on Dietetic Registration

Diabetes Educator Certification preferred

Salary Range: $75,000.00 To $85,000.00 Annually

RESPONSIBILITIES:

• Work with primary care and weight management providers to manage patients with nutrition related conditions
• Assess nutritional needs and food patterns of patients, families, and the community
• Educate and counsel children and adults on nutrition to improve and maintain health
• Plan and provide individual and group instruction based on patient needs assessments
• Develop nutrition educational materials and educate the community on nutrition topics in various formats in conjunction with the Health Education department
• Lead and coordinate with nutritionists to develop nutrition Quality Assurance program, Peer
• Review, and other quality improvement activities, update and review nutrition documentation template in EMR periodically
• Assist in supervisory and administrative duties of other nutritionists
• Assist other department heads in supervision and teaching of staff in delivering nutrition education and counseling
• Collaborate with care team to provide care management to patients with chronic diseases and high-risk patients.
• Organize nutrition events with nutritionists and other staff
• Assist with collection and analysis of obesity and diabetes data and reports
• Assist research team on grant applications, data collection and analysis and clinical publications
• Provide training for other health center nutritionists, healthcare interns and Project Ahead students
• Assist with validation of UDS report and other relevant reports
• Other duties as assigned

QUALIFICATIONS:

• Current New York State Certified Dietitian license and registration
• Certification by the Commission on Dietetic Registration
• Diabetes Educator Certification preferred
• Previous clinical diabetes management experience – preferably in community setting
