Job summary

All Seniors Foundation is seeking a Nutrition Educator (Home-Based) to evaluate clients’ nutritional needs and craft individualized meal plans for seniors in their homes. You will educate patients and families, coordinate with caregivers, and align with clinicians to support holistic health.

The role emphasizes in-home assessments, culturally sensitive planning, and collaboration with an interdisciplinary team to achieve optimal outcomes for older adults in the Los Angeles area.

Qualifications

All Seniors Foundation is committed to enhancing seniors’ health through proper nutrition. By providing personalized dietary guidance, we help elderly clients manage chronic conditions, maintain energy, and enjoy a healthier life at home.

As a Nutrition Educator (Home-Based), you will meet with seniors to evaluate their nutritional needs, create individualized meal plans, and offer guidance on healthy eating habits. You will collaborate with caregivers and other healthcare professionals to support overall well-being.

Key Responsibilities:

• Conduct nutritional assessments and identify dietary goals.
• Develop meal plans tailored to health conditions, preferences, and cultural factors.
• Educate patients and families on proper food choices, portion control, and meal preparation.
