Job summary

Discovery Senior Living is seeking an experienced Licensed Practical Nurse (LPN) to provide hands-on nursing care in a Phoenixville, PA community.

You will work nights (7pm–7am), 7 days biweekly with every other weekend, following individualized care plans, administering medications, and coordinating with physicians, families, and care teams to promote resident safety and well-being. Pay is $35/hour with full benefits.

Qualifications

  • Current LPN license (or RN as allowed by state).
  • Minimum 2 years nursing experience.
  • Experience in assisted living, long-term care, or skilled nursing preferred.
  • Strong clinical judgment and communication skills.
  • Ability to work nights, weekends, and flexible schedules.
  • Proficiency with electronic documentation systems.

Responsibilities

  • Provide nursing care per resident care plans, policies, and procedures.
  • Conduct ongoing assessments and monitor changes in condition.
  • Administer medications and treatments as prescribed and document appropriately.
  • Coordinate care with physicians, families, pharmacies, and other care providers.
  • Support health promotion, disease prevention, and wellness initiatives.
  • Ensure accurate, timely, and compliant clinical documentation.
  • Assist with care planning, service plan updates, and resident evaluations.
  • Collaborate with caregivers, wellness team members, and leadership to ensure high-quality care.
  • Respond to resident concerns, incidents, and emergencies with sound clinical judgment.
  • Support training and guidance for care staff.
  • Ensure community adherence to applicable regulations.
  • Participate in quality assurance, audits, and compliance activities.
  • Perform other duties as assigned.

As a Licensed Practical Nurse (LPN), you’ll play an essential role in enriching the lives of seniors by providing compassionate, high-quality nursing care while supporting resident safety, dignity, and well-being every single day.

Your Role

As the Licensed Practical Nurse (LPN), you provide hands-on nursing care in accordance with each resident’s individualized care plan. You support comprehensive health and psychosocial needs, monitor changes in condition, coordinate services, and collaborate closely with interdisciplinary team members to promote optimal resident outcomes in a community setting.

What You’ll Do
  • Provide nursing care to residents in accordance with established care plans, policies, and procedures.

Qualifications
  • Current Licensed Practical Nurse (LPN) license, or a Professional Registered Nurse as per state regulations
  • Minimum of two (2) years of Licensed Nurse experience required.
  • Experience in assisted living, long-term care, or a skilled nursing environment preferred.
  • Strong clinical judgment, problem-solving, and decision-making skills.
  • Effective organizational, time management, and communication skills.
  • Ability to manage multiple priorities and delegate appropriately.
  • Demonstrated commitment to seniors and person-centered care.
  • Ability to work nights, weekends, and flexible schedules as required by the community.
  • Proficiency with electronic documentation systems and basic computer applications.
  • Ability to respond to medical emergencies.
  • Exposure to bloodborne pathogens and infectious disease.
  • Ability to lift or assist residents as required by care plans.
