Essential Functions:

• Assist with activities of daily living such as personal hygiene, ambulation/transfers, dressing/shaving
• Prepare meals and snacks as instructed
• Accompany patients to errands or medical appointments
• Perform light housekeeping activities
• Follow patient-specific care plans including hospice support, medication reminders, and range of motion exercises
• Assist with blood glucose monitoring, bed-bound care, incontinence care
• Use lifting equipment like gait belts and Hoyer lifts
