Job Description

Case Manager responsibilities include: providing intensive care management for patients, developing individualized care plans, and collaborating with an interdisciplinary team, including physicians, to provide a comprehensive approach to care.
Case Managers will also: help minimize and prevent hospital readmission for patients with special needs, guide patients as they navigate screenings, treatments, and recovery, follow up with patients and families to ensure understanding of the care plan, utilize psychosocial expertise to serve each patient's unique needs while ensuring quality and continuity of care, and form strong bonds with patients, families, and multidisciplinary colleagues.

Qualifications

Mandatory Requirements:
Current New Hampshire RN License or Compact State License - Required in hand at the time of submission
Minimum 2 years current Acute Care Case Management experience - Required
Level I or II Trauma Center experience - Required
BLS/CPR (AHA) - Required
Current Med/Surg/Surgical/Critical Care/PCU experience - Highly preferred
Prior Travel experience - Highly preferred
BSN - Preferred
Teaching Hospital experience - Preferred
