Job summary

CVHCare is seeking a compassionate and organized Registered Nurse to join our home health team as an RN Patient Care Coordinator. You will help lead and coordinate patient care while ensuring exceptional service, compliance with regulations, and clinical excellence across visits and plans of care.

You will coordinate multidisciplinary care teams, schedule home health visits, and serve as a liaison among patients, families, providers, and caregivers.

Qualifications

• Active California RN license
• Current BLS certification
• Home health experience preferred
• Knowledge of Medicare regulations and OASIS preferred
• Strong communication, organizational, and computer skills
• Leadership or care coordination experience preferred
